Our Washington DC Parent & Student Meeting is on Wednesday, February 3, in the OFMS cafetorium, 7:00 PM - 7:45 PM. All of our Washington DC forms are electronic this year, with the exception of the medication form and the luggage form. Please click on the link below to complete all required forms. Forms must be completed online by Friday, February 5, 2016.

The medication forms are still in paper form and are attached. If you have any questions about medication please contact Mrs. Koenig, our Clinic Health Aide, at 440-427-6212, or by email at jkoenig@ofcs.net. Mrs. Koenig will contact our district registered nurse for consultation if additional information is needed. Medication and medication forms are due to the office by Friday, March 11, 2016.
